Tutorial on Patient Data entering in Ayushkeralam


Illustrated Tutorial on How to enter Daily Patient Data in Ayushkeralam website

My dear doctors, welcome to our website www.ayushkeralam.org, the first ever website in India designed to publish daily patient treatment data of over 200 Ayush NRHM Homoeopathic dispensaries in Kerala.

Entering daily patient data is our duty as well as our necessity because this will eventually help us to display our performance in front of the authorities and our employer (Ayush NRHM).

Updating daily dispensary data in our website is the only and most powerful way to prove that Ayush NRHM Medical Officers are doing much more and systematic work than the "others".

As a district web co-ordinator I know many of my fellow Doctors are facing difficulties in entering data in website. So keeping them in mind as well as all other brethrens in need, I have written this Illustrated tutorial for easy understanding and quick data entry possible.

Here I will show you how to login into your dispensary account and enter daily patient data.

For getting into the website either use google or directly type www.ayushkeralam.org in the address bar of your browser.See the image below. Click any image for enlarged view.

Google search result will be displayed as under:-

After entering the website, look for the Login link on the upper right hand side of browser, see image below:-

For Login enter Username and Password under Dispensary Login Only , see image below:-

Please note your username and password is as below:

Username - (has been sent to you by mail)

Password - (has been sent to you by mail)

You must change the default password after FIRST login into your dispensary account. See image below:-

On successful login you will reach the data entering page, like this :- see image below

In this form you can enter daily new case disease analysis and also the daily total of all cases, both old and new.

1. Click on the date box and a calendar will pop out, in which you can select desired date. See image below.

2. Click on the disease list box and select the disease condition from the drop down menu. See image below

3. Click on the Total box and enter the number of cases appeared on that specific day. See image below.

4. Click on the green ADD button on right side for entering the data into database. On doing so you will find the entered data displayed below under heading current record. See image below.

5. Repeat steps 1 - 4 each time and enter all the new cases one after the other and you will get the list of total new cases under current record for one particular date. (See image below).

Now if you find any error or mistake in entering data there are options for EDIT and DELETE in the current record against each entry.(See image below)

By clicking the edit button you will be able to modify the date, disease condition as well as the number entered earlier. If satisfied with the correction click UPDATE button. (See image below)

In the same manner you can delete a record by clicking the DELETE button adjoining that record. See image below.

Next you have to enter the total number of patients visited the dispensary on that particular day, both old and new cases. See image below.

Click add button after entering total cases. See image below.

On clicking add button your data will be added to the database and no longer will be visible on the form. For verifying the data entered click "View as Consolidate" button provided below. See image below.

A new pop - up window will open and show the detailed/consolidated entry. See image below.

Now click the "Edit & Submit Category" button for submitting your daily category total for upload. In addition to submitting your daily case total, by clicking this button you also have the privilege to EDIT or DELETE category total, if you find any discrepancy in it. See image below.

Make proper editing if any and when satisfied click the "Submit" button for data uploading.

On clicking the Submit button you will see a confirmation message "Submitted Successfully" a while after. See image below.

Now click the "Back" button to go back to the main "Patient Daily Record" entering page. See image below.

After reaching at the main "Patient Daily Record" entering page, now its time to upload your dispensary data for district and state admin verification. That is made possible by simply clicking the "SUBMIT" button, provided at the bottom of the form. See image below.

Please remember to click Submit button everytime you enter new data for a particular date. Only after clicking the submit button do your data becomes visible in website. So for having an overview of your daily performance by somebody you should SUBMIT the data daily.

Another importance of daily data entering is that your performance on "Monthly" and "Daily" basis can be evaluated by anyone around this world. For that click the link "PD" on the left hand side of the "Patient Daily Record" entering page. See image below.

In this "Performance Data" page you can view Patients detail through "Search by month" and "Search by date" method. See image below.

Please do try to enter data everyday so that you get familiar with the website and the data entering format. If any doubt contact your district web co-ordinator for detailed clarification and live demonstration of the same with computer.
